In Vitro Maturation (IVM)

IVM is a revolutionary fertility treatment that creates a safer and cheaper alternative for all couples who meet the requirements to undergo this therapy. In vitro maturation (IVM) is a new procedure similar to in vitro fertilization (IVF). The most significant difference between IVM and IVF is that in the in vitro maturation, the eggs are taken by the woman in an immature stage. This means that there is no need for the patient to be prescribed medication before the eggs harvested differently than what is done in the conventional IVF. In the VF, in fact, through a process called “induction of ovulation” they force more eggs to mature inside the body before being picked to be fertilized. To achieve this, the woman has to undergo a series of hormone injections that sometimes can be painful and with some side effects.

The IVM procedure is as follows:

As in traditional IVFs, eggs are collected at an earlier stage when they are in an immature stage. This means that you do not have to wait for the hormone stimulation of the ovaries before the eggs are harvested.

The eggs mature in a special incubator and remain in a laboratory container for one or two days.

When eggs are mature they are fertilized with sperm. Embryos grow and are then transferred to the uterine cavity just as is the case with the conventional IVF treatment.

IVM can be ideal for patients:

  • That are not able to tolerate the effects of the drugs needed to stimulate the maturing of the egg;
  • Who have been diagnosed with cancer and, in particular, in the early stages of recovery;
  • Who have ovarian disorders or Fallopian tube lesions;
  • With polycystic ovary syndrome (PCOS).

In December 2016 the first child that was born in Albania, under the supervision of Prof. Dr. Hakan Satirogly and Bio. Besfort Ferhati, and concieved by using the new technique, has been widely and efficiently applied by the Dental & Beauty clinical fertility staff.

Oocyte Cryopreservation (Egg Conservation)

Conservation of ovules (their freezing) is a process in which women’s eggs are stabilized and stored in a special container. Later, when they have matured, the eggs dissolve, they get fertilized and transferred into the uterus as embryos.

Egg preservation is recommended for these three groups of women:

  • Women with cancer diagnosis who have not yet started chemotherapy or radiotherapy;
  • Women who are subject to assisted reproduction techniques that do not use embryo freezing;
  • Women who want to preserve their future ability to have children, either because they do not yet have a partner, either for personal reasons or for medical reasons.

The reproductive period of women is limited and mainly depends on the number of eggs with which it was born and this unfortunately can not be improved with any therapy. Egg preservation is also an interesting option for those women who want to postpone maternity because of professional commitments.

Over the past 10 years, methods for quick egg freezing, or vitrification, have significantly improved egg survival after cryopreservation. The results of numerous studies show that frozen and fresh eggs give the same degree of pregnancy in in vitro fertilization cycles.

In November 2016 for an infertile couple in Italy who had chosen the Dental & beauty fertility clinic under Dr. Elton Peci and Bio. Besfort Ferhati has successfully performed pregnancy with eggs frozen from the IVF cycle.

OVARIAN TISSUE CRYOPRESERVATION (Oval Freezing)

It is an option to preserve fertility in patients who have to undergo chemotherapy or radiation therapy urgently, or patients in other pathological conditions requiring therapes that may affect ovarian function up to sterility.

Preserving ovaries is mainly used by these groups of women:

  • Women with aggressive malignancies who need immediate treatment in case there is enough time to allow the ovaries / embryos to freeze as a result of the induction of ovulation;
  • Women who have an autoimmune disorder that has already failed in immunosuppressive therapy may opt for ovarian conservation as a prophylactic option;
  • Women with genetic mutation leading to high risk of premature ovarian insufficiency;
  • Women with hormone-sensitive malignancy.

Most oocytes are located in the primordial follicles in the ovary cortex as a result of receiving a minimum volume of cortical tissue allowing storage of a large number of matured oocytes.

The most common method used to obtain ovarian tissue is laparoscopy. At the time when the ovarian tissue is obtained, it is transferred to the laboratory and a dissection of tissue fragments is made, that is 0.3 to 2 millimeters wide and ready to freeze.

The most effective method of freezing is the vitrification that eliminates the risk of the formation of ice crystals in the ovarian tissue.
